You’ve given a lot of thought into choosing a daycare provider for your child. So we are thrilled that you want to enroll your child at Texas Children’s Academy. Below you will find links to the required daycare enrollment documents. If you have already attended a daycare enrollment tour, and have received confirmation from our director that your child has been accepted for enrollment you may begin the paperwork process now. Please click on each link, print the forms, fill them out and return them to the daycare along with your registration and curriculum fee.

FORMS REQUIRED FOR ENROLLMENT:

TCA Daycare Enrollment Packet – All forms must be completed in their entirety!

TCA Daycare Parent Handbook – Please be sure to sign the “Receipt of Parent Handbook” page and include it in your paperwork.

IF YOU ARE ENROLLING MORE THAN ONE CHILD, PLEASE PRINT AN ADDITIONAL OF THE FOLLOWING FORMS FOR EACH CHILD:

State Enrollment Form – This form must be completed for each child. Please be sure to read the instructions on the form and fill in every field.
